If you are a new patient who hasn’t been seen in one of our offices before, please call to schedule your first appointment. You will find our new patient paperwork below. Please print it, fill out a copy, and bring it with you to your first appointment.

For existing patients, you are welcome to set up an account with Chirotouch. This feature is provided for your convenience. Chirotouch will allow you to schedule appointments, receive bills, and make payments online.
